2018 FSQAP Review Tool Notices

General Information: Revised Provider Discovery Review tools to be used beginning July 1st, 2019 have been posted.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2019_0701 indicates it is the July 2019 version. Please discard earlier versions of the tools with footer dates prior to July 2019 for the tools listed.

Please review Discovery tools in their entirety for services rendered to find updates to the standards, protocols, and Not Met reasons.

Update 1 of 1: The revised Support Coordination Tool and CDC+ Consultant Tool beginning January 1, 2019 have been posted.

Primary edits include:

• Changes made to standard #5 to address delay in required implementation of new Person Centered Support Plan format. Per APD, use of new Support Plan format is optional and required implementation date is still TBD.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2019_0101 indicates it is the January 2019 version. Please note this is the second edit for January so v.2 has been added after the date.

Please discard earlier versions of the tools that do not have this footer.

Primary edits include:

• Edited wording for an expectation under Health related to medications
• Combined two expectations under Safety
