General Information:

Update 1 of 1: The revised Support Coordination Tool and CDC+ Consultant Tool beginning January 1, 2019 have been posted.

Primary edits include:

  • Changes made to standard #5 to address delay in required implementation of new Person Centered Support Plan format. Per APD, use of new Support Plan format is optional and required implementation date is still TBD.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2019_0101 indicates it is the January 2019 version. Please note this is the second edit for January so v.2 has been added after the date.

Please discard earlier versions of the tools that do not have this footer.
