2018 FSQAP Review Tool Notices

July 1, 2022 ~ Update for WSC and CDC+ Consultant Qualified Organization Administrative Tool

General Information: Provider Discovery Review Qualified Organization Administrative Tool related to the CDC+ Consultant and Waiver Support Coordinator Reviews effective July 1, 2022, have been posted.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2022_0701 indicates it is the July 2022 version. Please discard earlier versions of the applicable tools with footer dates prior to July 2022.

Edits include:

  • Edit in the General Administrative protocol and addition of a not met reason in Standard #5 related to Business Liability Insurance.
  • Edits in the General Administrative protocol and not met reasons for Standard #9 related to the mentoring program requirements.
  • Edit in the General Administrative protocol in Standard #11 with reminder of requirement of WSC to be employees of the QO.
  • Edit in the Qualifications and Training protocol and not met reasons in Standards #6 for HIPAA, and, #7 for HIPAA, HIV/AIDS/Infection Control.
  • Standard #10 removed. While required for all existing WSC to maintain record of completion, this should be completed for all WSC hired prior to 6/30/2021 and therefore standard was removed. Training will be captured for WSC hired after 7/1/2021 under Standard #11 and this Standard was also updated to reflect the wording updates.
  • Standard #12 updates to protocol.
  • Please review all standards for wording updates in the protocol text and the not met reasons.

January 12, 2022 ~ Updated Operational Policies and Procedures

Qlarant has worked with the Agency for Healthcare Administration (AHCA) and the Agency for Persons with Disabilities (APD) to develop a plan to transition back to onsite and in person review activities.

While all entities continue to be mindful of risks associated with COVID -19, we have the tools and resources needed to help us move forward safely.

Updated Operational Policies and Procedures utilizing components from previous onsite review and desk review processes are now in effect.

November 15, 2021 ~ Update for QO Administrative Tool

General Information: A correction to the QO Administrative tool has been posted.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2021_1001_v4 indicates it is the October 2021 version 4. Please discard earlier versions of the applicable tools with footer dates prior to October 2021 v.4.

QO Administrative Edits include for version 4 (v4):

  • Standard # 7 (The provider follows their approved Mentor Mentee program.) under General Administrative has been removed from the tool.

General Information:

Qlarant Desk Review Procedures developed at the start of the COVID-19 pandemic have been updated.

Updates include:

  • The addition of a section addressing how iConnect documentation will be handled during the desk review process
  • Sampling for WSC personnel files
  • General edits to other sections

October 15, 2021 ~ Update

October 15, 2021 Update for Qualified Organization (QO) Administrative and Provider Administrative Tools

General Information: A correction to the Administrative Tools has been posted.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2021_1001 indicates it is the October 1, 2021 version. Please discard earlier versions of the applicable tools with footer dates prior to October 1, 2021.

QO Administrative Edits for version 3 (v3) include the following:

  • Under Qualifications and Training > Standard #1 > Not Met Reason #19 – The (A) indicating reason is associated with an Alert was left off by mistake. This has been corrected.

Provider Administrative Edits for version 7 (v7) include the following:

  • Standard #46 under Qualifications and Training for Supported Living Coaching omitted the word “not” for not met reason #1. This has been corrected.

Under Qualification & Training section, for standard # 10, in the Protocol text, the number of attempts before APD will terminate the MWSA was corrected from two to three.

General Information: Provider Discovery Review Tools related to the CDC+ Consultant and WSC Reviews effective October 1, 2021 have been posted.

  • Tool was broken out into its own Qualified Organization Administrative Tool based on updates for implementation of Senate Bill 82 from the other service provider Administrative tool.
  • Tool has been separated into two distinct sections: 1) General Administrative, numbered 1-11 and 2) Qualifications & Training, numbered 1-15.
  • Policies & Procedures have been removed. Please note, while Qlarant will no longer be reviewing for Policies & Procedures, Agency Providers are still expected to develop and maintain applicable Policies & Procedures for their organization as identified in applicable Handbook, Rules, and Statutes.
  • New standards in the General Administrative section include standards # 5,6,7,8,9,10,11.
  • New standards in the Qualifications and Training section include standards #10,11,12.
  • Please review all standards for wording updates in the protocol text and the not met reasons

Provider Discovery Review Administrative Tool has been updated to be used beginning July 1, 2021 and posted with the following edits:

  • Tool has been separated into two distinct sections: 1) General Administrative, numbered 1-9 and 2) Qualifications & Training, numbered 1-49
  • Policies & Procedures have been removed. Please note, while Qlarant will no longer be reviewing for Policies & Procedures, Agency Providers are still expected to develop and maintain applicable Policies & Procedures for their organization.
  • Standards and all references to Waiver Support Coordination have been removed. A separate Administrative Tool for Waiver Support Coordination effective 10/1/21 will be coming soon.
  • Wording updates/clarifications to the protocol text in Qualifications and Training standards #1, #3, #4, #10, #11, #12, #14 and #15.
  • Not Met reasons added for Qualifications and Training standards #3, #4 , #9, #10, #11, #12 & #13

 

Update 2 of 2
Provider Discovery Review Service Specific Record Review (SSRR) Tools for Supported Living Coaching and Residential Habilitation (Standard, Behavior Focus, Intensive Behavior and Enhanced Intensive Behavior) have been updated to be used beginning July 1, 2021 and posted with the following edits:

  • An existing ANE standard has been broken out into two separate standards. Below are the two standard numbers for each service tool:
    • Supported Living Coaching – standards 22 & 23
    • Residential Habilitation Standard – standards 18 & 19
    • Residential Habilitation Behavior Focus – standards 22 & 23
    • Residential Habilitation Intensive Behavior – standards 23 & 24
    • Residential Habilitation Enhanced Intensive Behavior – standards 22 & 23
