Revised Checklists – September 17, 2026

Updates have been made to all Administrative and Service Specific Record Review (SSRR) Checklists.

Updates include:

    • Addition of examples and explanations
    • General formatting edits

To ensure you have the most current version, the footer of each checklist will reflect the date. For example, the 2026_0901 indicates it is the September 2026 version.

Please discard earlier versions of the applicable checklist with footer dates prior to September 2026. Please review the Administrative Tool and Service Specific tools in their entirety for standards, protocols, and Not Met reasons.
