June 17, 2026 – Provider Review Tool Updates

Updates have been made to Provider Discovery Review Tools. These updated tools are now posted.

Please see below for changes.

PDR CDC Representative Tool

  • General edits to protocols
  • New Standard #7 for CDC+ Representative Background Screening added with applicable protocol and not met reasons.
  • Renumbering of Standards after new standard added.
  • Standard #8 (Background Screening for Directly Hired Employees): Five Not Met reasons deleted. Two Not Met reasons added. Other Not Met reasons edited. Related protocol updates.
  • Standard #9: Two Not Met reasons deleted. Two Not Met reasons added. Related protocol updates.
  • Standard #10: Not Met reasons and protocol updated accordingly.

 

Life Skills Development 3 (LSD3) and Life Skills Development 4 (LSD4) Tools

The LSD3 and LSD4 Service Specific Record Review tools have been updated to come in line with the rule change requiring these services be billed monthly or hourly based on number of hours of service provided per month.

  • Protocol for Standard #1 has been updated and two Not Met reasons added.
  • Protocol for Standard #12 (formerly #13) and Not Met reasons updated.
  • Standard #10 deleted

 

Duplicate Notes

New Standard: Service Logs document service delivery that is varied, unique and person centered.

This new standard with applicable protocol and not met reasons has been added to the following Service Specific Record Review tools. It is the last standard on each tool: Behavior Analysis, Behavior Assistant, LSD1, LSD2, LSD3, Personal Supports, Respite and Supported Living Coaching.

To ensure you have the most current version, the footer of each Discovery Tool will reflect the date. For example, the 2026_0701 indicates it is the July 2026 version.

Please discard earlier versions of the applicable tool with footer dates prior to July 2026. Please review each Discovery Tool in its entirety for services rendered for standards, protocols, and Not Met reasons.
