July 1, 2021 ~ Update

Update 1 of 2:
Provider Discovery Review Administrative Tool has been updated to be used beginning July 1, 2021 and posted with the following edits:

  • Tool has been separated into two distinct sections: 1) General Administrative, numbered 1-9 and 2) Qualifications & Training, numbered 1-49
  • Policies & Procedures have been removed. Please note, while Qlarant will no longer be reviewing for Policies & Procedures, Agency Providers are still expected to develop and maintain applicable Policies & Procedures for their organization.
  • Standards and all references to Waiver Support Coordination have been removed. A separate Administrative Tool for Waiver Support Coordination effective 10/1/21 will be coming soon.
  • Wording updates/clarifications to the protocol text in Qualifications and Training standards #1, #3, #4, #10, #11, #12, #14 and #15.
  • Not Met reasons added for Qualifications and Training standards #3, #4 , #9, #10, #11, #12 & #13

Update 2 of 2
Provider Discovery Review Service Specific Record Review (SSRR) Tools for Supported Living Coaching and Residential Habilitation (Standard, Behavior Focus, Intensive Behavior and Enhanced Intensive Behavior) have been updated to be used beginning July 1, 2021 and posted with the following edits:

  • An existing ANE standard has been broken out into two separate standards. Below are the two standard numbers for each service tool:

    • Supported Living Coaching - standards 22 & 23
    • Residential Habilitation Standard - standards 18 & 19
    • Residential Habilitation Behavior Focus - standards 22 & 23
    • Residential Habilitation Intensive Behavior - standards 23 & 24
    • Residential Habilitation Enhanced Intensive Behavior - standards 22 & 23

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2021_0701 indicates it is the July 2021 version. Please discard earlier versions of the applicable tool with footer dates prior to July 2021.   • Added and edited standards related to new Med Administration Rule that went into effect July 1, 2018, including five new standards

The Implementation Plan webinar recording presented by Qlarant and Empower Florida is now posted.

Please Note: Training certificates were issued by Empower Florida for those who registered and attended the live 11am and recorded 5pm sessions on June 25, 2020 only.

Those who view this recording after June 25th will not receive in-service training credit hours nor a certificate. Please do not contact Qlarant about receiving a training certificate.

Edits include wording updates to the protocol text in Standard #1 and #2 related to the timeframe for the Medicaid Eligibility Worksheet effective 1/1/2020.

Update 1 of 1: The revised Support Coordination Tool and CDC+ Consultant Tool beginning January 1, 2019 have been posted.

Primary edits include:

  • Changes made to standard #5 to address delay in required implementation of new Person Centered Support Plan format. Per APD, use of new Support Plan format is optional and required implementation date is still TBD.

To ensure you have the most current version, the footer of each Discovery tool will reflect the date. For example, the 2019_0101 indicates it is the January 2019 version. Please note this is the second edit for January so v.2 has been added after the date.

Provider Discovery Review (PDR) Survey

Request for Reconsideration(updated May 1, 2018)
If you disagree with the findings related to noted billing discrepancies in your Provider Discovery Review (PDR) report, you may request a Reconsideration Review.  The Reconsideration Request must be made in writing(using the Reconsideration Request form located below) and recieved within 30 days of the annual PDR report mailing date.

The National Core Indicators is a collaboration among participating National Association of State Directors of Developmental Disability Services (NASDDDS) member state agencies and the Human Services Research Institute (HSRI), with the goal of developing a systematic approach to performance and outcome measurement.
